Sample Course: SUBMIT Ministry Practicum

MINISTRY PRACTICUM is required of all undergraduate and master’s student’s taking courses for college credit. It ensures that a student receives proper credit for fulfilling his/her obligation to serve a minimum of 72 hours per year in the ministry of the church he/she attends. This equates to 2 hours per week. It is the student’s responsibility to ensure that this form is completed and submitted to the Dean / Professor no later than the 28th of each month. Fill out one (1) line of section #3 per job performed. Please keep a copy of the completed form for your records.

You are required to submit hours (8) of Ministry Practicum.

A MAXIMUM OF 2 HOURS A WEEK WILL BE ACCEPTED ON YOUR MINISTRY PRACTICUM ACTIVITY LOG. You are however encouraged to exceed the maximum hours per week in order to personally learn and grow in different areas of ministry.

Please download, fill out, and upload the completed form in the form of a PDF document. A Supervisor signature is required. Once uploaded, your completed form will be subject to approval by your assigned Professor. You may use free apps called Genius Scan or CamScanner to scan your activity log and download as a PDF.
